Intervention Presentation and Capstone Video Reflection

Good afternoon everyone! My name is _____ and I thank all of you for joining me for this video presentation. Today, I’ll present the evidence-based intervention that I developed and implemented in my capstone project. This intervention plan was developed for patient Joey who suffered from coronary heart disease and faced problems in acquiring adequate care treatment. Let’s see how this intervention plan played an effective role in improving Joey’s condition.

Contribution of an Intervention to Patient Satisfaction and Quality of Life

Before the proposed intervention for CHD treatment and management, Joey had to make multiple revisits to hospitals due to complications of his condition. It was found he had problems related to healthcare provision at the hospitals and some financial challenges also prevented him from treating his condition effectively. The proposed plan was a telehealth-based self-management education program for CHD along with the remote monitoring of medication adherence to promote medication compliance. This intervention was developed considering all these factors which were problematic to Joey and acted as a barrier to manage his disease. This patient-centered intervention was effective and resulted in enhanced patient satisfaction and quality of life. 

Joey described that remote monitoring of medication intake was a great help in taking medication consistently. It made him become more mindful of the next dose to be taken and this application of notifying the nurse on medication adherence increased his motivation to take medication. The nurse’s recommendation of using a smart pill box to divide medication into each box for a whole week was excellent to prevent from forgetting to take medication.

Joey also mentioned that the self-management education program helped him observe a healthier lifestyle and maintain consistency as well. He was more aware of what foods he should eat and how physical activity can help him alleviate the progression of his condition. These interventions helped Joey control the risk factors of CHD and manage his condition effectively. This ultimately increases their quality of life, experience, and satisfaction. 

Patient safety and quality of care are two key factors in providing healthcare services. When these factors are achieved, patient experience leads to patient satisfaction and improved quality of life. My proposed intervention fulfilled all these criteria as the intervention developed was patient-centered and involved nursing ethics. The needs of the patient were kept under consideration and clinical-decision making was done after noting the patient’s concern to his intervention. Hence, Joey’s experience was enhanced and his satisfaction and quality of life were improved through this proposed intervention.

Evidence and Peer-Reviewed Literature to Plan and Implement a Capstone Project

Evidence-based practices (EBP) are significant in designing and implementing an intervention for a capstone project. As these practices improve the clinical outcomes in the capstone project. Amalgaming evidence-based practices and credible data with the capstone project learnings, and interventions obtained can improve the clinical results of the patients (Kim et al., 2019). In my capstone project’s planning and implementation, evidence and peer-reviewed literature were utilized. These studies provided clear evidence of what strategy to include and how it can help my patient’s condition. 

According to the American Heart Association, telehealth plays a great impact on the effective management of cardiovascular diseases including CHD (Takahashi et al., 2022). This supported the utilization of telehealth in bringing improvements for cardiovascular patients. Another study utilized the mobile-health for the self-management of cardiovascular patients by providing them with tools and guidelines to control their risk factors. This included lifestyle modifications, and the use of wearables to monitor their vital signs and heart rate. Nurses used smartphone apps or website-based patient portals to provide care treatment to patients in the comfort of their homes. This intervention in cardiovascular patients improved their overall quality of health and also well-managed their condition (Cruz-Ramos et al., 2022). 

Another study on the utilization of mobile health technology for monitoring the medication adherence in patients to improve medication compliance using educational tools to promote medication adherence and deliverable reminders for their study population. This study resulted in improved outcomes in the overall health of the patients by promoting medication adherence through smartphone apps (Al-Arkee et al., 2021). Such evidence-based literature influenced me to create likewise intervention i.e. telehealth-based self-management education program and medication adherence application for Joey to improve his compliance to his medication regimen and bring quality improvements in his health condition.

Leveraged Health Technology in Improving Outcomes and Communication in Capstone Project

 For effective communication and gaining desired improved outcomes in a capstone project, it is essential to incorporate health technologies in all steps of providing care treatment. Healthcare technologies like information exchanging social media apps such as WhatsApp and telephonic calls and video calls can be a few examples to make articulate and effective communication among patients and health practitioners (Lee et al., 2020). In my capstone project, the role of health information technologies was the highest. From communicating with other health team members to providing proposed interventions, health information technologies were utilized throughout this capstone project.

These technologies included telehealth for communication with patients to provide treatment and management of CHD. Other than that, smartphone applications were used to convey patient data on medication adherence, this also served to be a means of communication with my patient and his family. These communication tools of health information technologies were a great source of clinical improvement in patient outcomes. Further prospective opportunities for improving healthcare technologies can be made by publicizing the use of health information technologies. This can be done by increasing the public’s literacy levels about the impact of healthcare technologies on health problems. This will enhance the use of technologies like wearables, fitness trackers, wellness apps, reminders for physical activity, and other lifestyle changes in the everyday life of patients with chronic diseases (Tricás-Vidal et al., 2022).

Influence of Health Policy on Planning and Implementation of Capstone Project

Various policies have influenced the planning and implementation of this intervention in this capstone project. One such health policy is the Affordable Care Act. This Act promotes the enhanced access to health services, especially for people who come from low-socioeconomic backgrounds and are unable to afford care treatment and services (Wolfe & Joynt Maddox, 2019). This policy influenced the intervention of telehealth as telehealth is developed with the same purpose of improving healthcare accessibility and reducing health inequities by overcoming social and economic barriers. 

Another health policy that influenced the planning and implementation of my capstone project is HITECH Act. This Act stresses the utilization of healthcare information technologies in healthcare systems to enhance patient satisfaction by improving clinical outcomes (Kadakia et al., 2021). This influenced my incorporation of the telehealth-based self-management education program and remote monitoring of medication adherence. By combining treatment with health technology, clinical outcomes are enhanced and take place progressively.

The capstone project based on these health policies contributed to developing some new health policies in my organization, whereby telehealth technologies were introduced for patients with chronic diseases who had limited access to healthcare due to social, economic, and geographical barriers. Moreover, an organizational policy was developed for these patients to strive to reduce their healthcare disparities by enhancing access to healthcare services.  

The role of nurses in developing policies includes exploring those health policies which are most beneficial for the healthcare provision, seeking those legislators who support those policies, writing to their legislator about problems that affect care treatment, sharing awareness among other fellow healthcare staff about the opportunities which can impact policy change (Hajizadeh et al., 2021). By following these steps, nurses will identify the healthcare issues that may require the development of organizational policies for improving the quality of care. They will communicate and collaborate with organizational stakeholders to develop these policies. Once the policies are developed with the efforts of nurses, they will ensure that policies are implemented for better provision of care by using effective communication and collaboration with healthcare providers.

Outcomes of the Project 

The main focus of this capstone project was to enable Joey to get better healthcare treatment and management by raising awareness about his condition and management strategies. Joey was initially unaware of the community resources and health policies he could have utilized for achieving better health outcomes. He also lacked knowledge about his condition and showed medication non-adherence due to various reasons. The capstone project initially designed for Joey was based on the prediction that Joey will improve his CHD condition by enhancing self-management education and medication adherence. 

After implementation of the proposed intervention, the outcomes matched the initial predictions as he was showing better health outcomes and his disease was under control. This proposed intervention can be adopted as the best practice for the management of CHD not only in this hospital organization but outside this particular setting as well. Since the final outcomes of Joey’s condition are the desired outcomes in any patient with CHD, this intervention can be generally practiced in other hospitals having patients with similar conditions. The practicum hours spent with Joey and his family have been clearly documented in the Capella Academic Portal Volunteer Experience Form with appropriate information gathered from these sessions. 

Professional Growth Throughout a Capstone Project

Throughout the capstone project, it was made sure that ethical care provision is delivered. The ethical care provisions followed in this capstone project were autonomy, beneficence, non-maleficence, and justice. It was kept in consideration that Joey has the right to make his own decisions based on his beliefs and values. His needs and beliefs were respected throughout this project. The treatment provided was free from any hatred, maltreatment, and harm. Joey was facilitated at all times when he was unable to perform his task. He was treated in a just manner and without any maleficence. The nursing practices were provided with empathy and compassion. His safety and health were considered the most important parameters to manage. In short, Joey was provided care with the best professional standards. 

As I worked with Joey on this capstone project, I was continuously growing professionally. But the part I am most proud of was the fact that I was able to provide Joey with the best possible healthcare as per his needs and that his disease was under control. This has enhanced my practicing satisfaction and motivated me to be more considerate towards patients like Joey and provide them with the best possible care treatments. Besides professional growth, I also grew personally. This capstone project made me reflect on how my lifestyle can also become problematic later in my life. Therefore, I adopted healthy habits such as eating healthy meals and exercising daily. This helped me become more active, energetic, and healthy not only physically but mentally as well. This physical and mental health will further help me perform my nursing practices better in the future. 


To conclude, we have covered the intervention presentation and capstone video reflection. In this presentation, we talked about the contribution of the proposed intervention to Joey’s health and learned how this intervention enhanced Joey’s experience. Afterward, we covered some evidence and peer-reviewed literature used in the planning and implementation of this capstone project. We also found to what degree healthcare technology helped in improving outcomes and communication with Joey. Moreover, we discussed health policies that impacted the planning and implementation of this capstone project and the contributions this capstone project made to policy development. Lastly, we analyzed the outcomes of this capstone project to the initial predictions.
