They are factors that include demographics, environmental considerations, and external policies and systems (World Health Organization, n.d.). Although limited healthcare services are available within the town, they can at least provide primary care services to the population to prevent further delays in the management of diseases. Community leaders and the local population have strong bonds and showed resilience and support, which will act as a strong asset for health promotion initiatives.  Opportunities to Improve Some of the areas of improvement include health awareness, inclusion of nutritional food in shelters and restaurants, introducing health curricula in schools, and growing healthcare facilities within the town. This proposal promotes the implementation of community awareness campaigns led by non-profit organizations in the community and school-led awareness sessions. Moreover, developing two small rehabilitation centers with trained individuals is proposed as another strategy to eliminate the concern from the community. The goal of this program is to engage the community by utilizing their human and materialistic resources to prevent such negative cases within the area. This idea is supported by the research that the engagement of community and community resources has proven effective in addressing the opioid crisis and improving health outcomes (Sprague Martinez et al., 2020). Furthermore, the idea is to prevent high-risk children so that a healthy community is fostered and rehabilitate addicted individuals to prevent the spreading. To foster stakeholders’ understanding of the proposed changes, a comprehensive strategy is needed, enabling them to fully apprehend the change proposal and evaluate the existing healthcare systems for successfully buying the ideas. This strategy comprises interactive workshops and presentations for stakeholders, including healthcare providers, project managers, and community leadership. Through these strategies, stakeholders will be aware of the proposed changes, their benefits, and the potential challenges that may arise during the implementation. They will also be instructed to address those challenges through graphical representations of global statistics and success stories in other healthcare facilities. To evaluate the proposed changes, stakeholders may need a monitoring strategy where the prevalence of mental health and addiction issues will be measured in children. The effectiveness of the programs will be monitored by the community’s feedback, decreasing the healthcare burden and improving the quality of life.  Information Needs of the Decision-makers:  Decision-makers may require specific information on cost-benefit analysis, which includes the short-term and long-term financial needs and expected return on investment. Moreover, potential risks associated with the proposed strategies and a mitigation plan will also be the information requirement of decision-makers. This is performed through community health assessment, described by the Centers for Disease Control and Prevention (CDC) as a structured approach to analyzing public needs, developing interventions and programs, and executing them to address specific concerns. This community health assessment focuses on the Jordan community, which is a small town in the Minnesota countryside. The methods used for the identification of health needs of the community care windshield surveys and environmental analysis. Executive Summary General Condition and Public Health Needs The windshield survey conducted for Jordan, which is a small town in Minnesota identified a City Hall, which was built in 1885 and fulfills the motto of Jordan city, “warm country feeling, minutes from the cities,” This hall is used as a venue for various events and programs within the city. Next, we see some schools almost in nearby to each other. Jordan’s middle school is adjacent to the Community Education and Recreation Center (CERC), and it is a student and family-friendly campus. Moreover, the city has a high school and an elementary school with respective subjects for specific grades, and a public library is founded as well. #The survey reveals a social determinant that most people in Jordan undertake post-secondary education. Other than the educational sector, we observed many refugee camps throughout our drive which were managed by Jordan’s government and private partnerships together. Along with this, we drove by a food shelter which is run by 100 volunteers, feeding 20 families per weekend. Additionally, we observed a health services center that is 10 minutes away from the town. The center provides services for outpatient clinics, inpatient, and emergency care. Other tertiary care hospitals are available outside Jordan town by 15 mins drive. NURS FPX 6218 Assessment 2 Assessing Community Health Care Needs Within the area, there are small dentistry, chiropractic, and family medicine clinics available. The beauty of the town lies in its widespread recreational parks, dine-in areas, and shopping streets, for which it is famous. Our survey identified certain areas with crowded streets due to shopping stores and large buildings, which were negatively impacting the traffic and transportation system. It is estimated that the traffic volume will significantly increase by 2025. Some of the public health needs identified were healthcare accessibility and preventive care services as tertiary hospitals are away from the town. Moreover, health disparities were observed due to a diversified population, which also led to mental health problems. The limitations of our survey included a lack of resources, limited time, and the reluctance of people to provide detailed information.  Environmental Analysis The environmental analysis of the Jordan community is performed using the SWOT technique. This is an acronym for strengths, weaknesses, opportunities, and threats. It is an analysis used to evaluate the internal and external factors of an organization or community (Teoli et al., 2022). The survey and the analysis of environmental factors in the Jordan community identified several health needs of the population. Some environmental factors that affect the population’s health are a rapid increase in the number of residents, limited healthcare facilities, growing traffic and transportation problems, and excessive dine-out places, which leads to poor nutrition. According to Universal Health Coverage, undocumented refugees can access healthcare services in their residence country (Onarheim et al., 2018). However, the rising population in Jordan is leading to healthcare disparities, significantly impacting the refugees, as limited healthcare coverage is implemented for the underserved population. Another environmental factor that impedes population health is the increasing demand for business and residential areas, which impacts the traffic and transportation system of the town. The increasing traffic volume can lead to pollution and other health risks like cardiopulmonary problems. Population facing these health problems may require emergency services unavailable within the town and may require commuting to tertiary hospitals, which can increase the financial burden on the individuals, delay diagnosis and treatment, limit access to tertiary care services, and, in severe cases may increase mortality rate. Lastly, the excessive number of restaurants in Jordan town gives rise to poor nutrition as a population health concern. This food may contain unhygienic contents, which may lead to several diseases. Studies have revealed that there are increasing risks associated with the food supply chain due to inadequate food safety checks and the use of poor nutritional items (Gizaw, 2019).  Social Determinants of Health  The critical social determinants of health for the Jordanian community include population, housing facilities, employment, education, and healthcare access. The current population of the community is about 7000 people, with about 2500 housing facilities. Along with this, the employed individuals are around 2200. With the predicted rising population to reach 12,000 in 2040, the housing facilities are expected to reach 4700, with only a minimal increase in employment projections. This suggests that increasing population may lead to negative outcomes on individual employment and the overall economy of the community.Limited financial resources may impede the process of healthcare accessibility for community members as individuals may be prevented from receiving care. Moreover, this may also prevent them from utilizing insurance packages, ultimately building up healthcare costs (Coombs et al., 2022). As far as education is concerned, 34% of the population are high-school graduates (City of Jordan, Minnesota, 2015). The high level of education within the community is connected with increased literacy and awareness about health issues, which plays a significant role in improving the community’s health. In terms of healthcare access, the community is facing challenges due to the increasing population and limited available services. These challenges may lead to growing healthcare concerns within the community. The assumption of this analysis is that increasing population limits the available resources. However, the uncertain points are pandemic/epidemic situations, which may further aggravate the community’s resource needs, impeding smooth processing.  Conclusion  In conclusion, we identified various
